The following list may be useful to students and preceptors who are planning new pharmaceutical care services.
Resources for project planning
The following website contains a listing of national health observances (example: National Breast Cancer Awareness Month in October) as well as organizations to contact for free educational resources. On the website for the North Carolina Association of Pharmacists (NCAP) there are useful tools for planning an influenza vaccination clinic. They have a sample protocol that can be downloaded and adapted to a particular pharmacy, as well as a copy of the board rules allowing pharmacists to administer vaccines.
The Virginia Pharmacists Association website also has a downloadable sample protocol at http://www.vapharmacy.org/vpha/ , and click on "VPhA Model Protocol for Immunizations". This is posted as a word document so that pharmacists may customize it for their pharmacies. Virginia immunization laws differ from North Carolina's laws, so this protocol must be edited to reflect these differences (In North Carolina only influenza and pneumoccocal vaccines for adults may be included in the immunization protocol).
This newsletter offers project ideas, ideas for student activities, suggested readings, discussion topics, and other resources for the student and preceptor.
This calcium calculator located on the TUMS website is a good resource for students planning a women's health service or an osteoporosis education service. It may be used to calculate how much calcium the patient needs to get their daily requirement.
The FDA has several publications, brochures, and ads that may be helpful when planning an OTC consultation service.
AJPE has several very useful articles relating to education in the community pharmacy setting. The above link is to an article that explains how to help students create their own disease state management and wellness services in the community pharmacy setting.
The Be Medwise website offers many tools to improve patient safety with over-the-counter medications. The website offers patient safety brochures and posters, and each visitor to the website may order a free patient safety tool kit.
The National Association of Chain Drug Stores (NACDS) Foundation offers a free National Health and Community Events Resource Guide that contains all of the national health observances and links to websites with resources relating to the health observances. Students may choose to base their project on a national health observance. For example, since October is National Breast Cancer Awareness Month, the student may wish to start a patient education service on preventing breast cancer. Free self breast exam cards may also be given out at the pharmacy during the month.
Smoking Cessation Materials
The Rx for Change website ( http://rxforchange.ucsf.edu ) contains free materials for teaching a class on smoking cessation, as well as other resources that could be used when implementing a smoking cessation service. Email me at sspencer@wingate.edu to get the student user name & password to get into Wingate's account online.
ASHP also offers many free materials for pharmacists who are establishing smoking cessation services. Go to www.ashp.org/s_ashp/bin.asp?CID=2038&DID=6458&DOC=FILE.PDF to get patient handouts for your smoking cessation service, or go to www.ashp.org/s_ashp/quart2d.asp?CID=504&DID=546 for other training programs & resources for smoking cessation services. The ASHP program is designed to train pharmacists in smoking cessation with a CE program and then give them the tools they will need to start their own smoking cessation service. The article (link above) on Medication Therapy Management defines Medication Therapy Management and includes forms to be used to create a patient medication record as well as a pharmacologic plan for the patient. These would be helpful for the student or preceptor developing their MTM services for patients with Medicare Part D. Also see http://www.accp.com/position/mtms.pdf for an excellent review of MTM as well as fee schedules for different types of MTM services.
To sign up to provide MTM services for Humana patients go to http://apps.humana.com/prescription_Benefits_and_services/execreq.asp?processcode=4005&srcsite=physician&src=redirect and click on "Medication Therapy Management (MTM)", continue through the prompts if you qualify, and print out the application to provide MTM services. Complete the form and fax it to 502-580-2200. The site also contains helpful information and guidelines on setting up your MTM services.
If a student wants to perform a stroke risk assessment screening they can develop their own screening forms or order forms from the National Stroke Association (see above link). Other stroke risk assessment resources also available at a very low cost.
You can order a free CE program for pharmacists on the topic of adult incontinence. Patient brochures are also available.
MORE RESOURCES:
ePocrates online has a free bone health assessment tool available for download. The tool contains risk factors for osteoporosis, treatment recommendations based on T-score results, as well as an explanation of T-score results. Download available to PDA's at www.epocrates.com.
CLIA WAIVER INFORMATION:
If your pharmacy provides lab screening/testing such as blood glucose, hemoglobin A1C, or cholesterol measurements with a cholestech machine you must register to become a CLIA waived laboratory. The link to the application form is:

There is a fee of $150 every 2 years to register your pharmacy to provide these CLIA waived lab tests.
If you have a pharmacy student that wants to provide blood glucose screenings or any other blood tests you must register to be a CLIA waived facility.
